American Traveler is seeking an experienced RN Case Manager with a BSN, EPIC experience, and prior travel assignment for an adult acute care environment.

\n \n Responsibilities\n

• Work in a hospital setting with 182 licensed beds
• Serve adult Medical/Surgical, Cardiac, and Orthopedic patient populations
• Typical caseload is 14-16 patients per day depending on assigned unit
• Requires experience with placement for SNF, IPR, LTAC, Hospice, Home Health, DME, and Behavioral Health needs
• Uses EPIC EMR and Rover Chat for communication; experience with Careport is preferred
• 8-hour day shifts, Monday through Friday, starting at 0800
• Weekend rotation will depend on assignment specifics
• Possibility to float to any unit as needed
• No on-call requirements
• Holiday coverage expected unless time off is approved in advance
• Daily responsibilities include care coordination, discharge planning, and working with complex patient populations including the homeless
• Assigned to a supportive, team-oriented department with managers, educators, and peer CMs providing guidance
• Department fosters a culture of communication, collaboration, and ongoing education
• Department orientation ranges from 2-5 days, adjusted to individual skillset
• Scrub color requirement is navy blue for RNs
• Free parking is available

• Active NC or compact RN license required
• Bachelor of Science in Nursing (BSN) required
• Current BLS certification required
• Minimum 3-5 years of recent Case Management experience required
• Prior experience as a travel RN Case Manager required; first-time travelers will not be considered
• EPIC charting experience is required
• Care management skills including assessment, coordination, and discharge planning are required
• Case Management certification preferred
